OVERVIEW

Your access to the Revelation Pharma mobile application and websites, (collectively, the “Revelation Pharma Digital Services”) is subject to the Revelation Pharma Terms of Use and this Privacy Policy. “Revelation Pharma,” “we,” “us,” and “our” mean Revelation Pharma, LLC and the affiliated and subsidiary pharmacies and outsourcing facilities it owns, which are identified at revelationpharma.com. Each pharmacy may keep its own separate website and terms; this Privacy Policy governs the Revelation Pharma Digital Services, except for protected health information, which is governed by our HIPAA Notice of Privacy Practices. This Privacy Policy applies only to information collected through the Revelation Pharma Digital Services and not to information collected offline.

Your privacy is important to Revelation Pharma. This Privacy Policy explains what information we gather from you when you use our services, how we may use that information, the security approaches we take to protect your personal information, and how you can access and update certain information that we collect. This Privacy Policy is incorporated and made part of the Revelation Pharma Terms of Use.

ACCEPTANCE OF PRIVACY POLICY

By using the Revelation Pharma Digital Services, you consent to the collection and use of your information by Revelation Pharma in accordance with this Privacy Policy. If you do not agree to this Privacy Policy, you may not use the Revelation Pharma Digital Services.

You understand that if you’re using the Revelation Pharma Digital Services from a country outside the United States, you will be transferring personal information about yourself to the United States. You understand that the privacy laws of the United States may not be as comprehensive as those in your country, and you agree that the transfer of your personal information occurs with your consent. Personal information collected by the Revelation Pharma Digital Services may be stored and processed in the United States or abroad.

MODIFICATION OF PRIVACY POLICY

Revelation Pharma reserves the right to change this Privacy Policy at any time and without prior notice. Changes take effect on the date that appears on the revised Policy. If you use the Revelation Pharma Digital Services following a change in the Policy, your use represents your acceptance of the changes. You are responsible for frequently reviewing this Privacy Policy for changes. We will post the revised Privacy Policy with a new effective date. Material changes will not apply retroactively to information already collected in a way that is inconsistent with the Privacy Policy in effect when it was collected.

WHAT INFORMATION WE COLLECT

You will be asked to provide personal information when you register your Revelation Pharma Digital Services account, place an order, respond to communications such as e-mail, or make use of another Revelation Pharma Digital Services feature. We may ask you for your name, e-mail address, mailing address, phone number, credit card information, or other information. Revelation Pharma uses this information consistent with this Privacy Policy, and we may also combine it with other information to provide and improve our services and content. You are not required to provide the personal information that we have requested, but if you chose not to do so, we may not be able to provide you with certain services or respond to certain inquiries you may have. We use IP addresses and session identifiers to analyze trends, to administer Revelation Pharma Digital Services, to track how our Digital Services are used, and to understand usage trends. We use cookies and local storage to enhance your experience and gather information about visitors and visits. Please refer to the “Cookies” section below for information about these mechanisms and how we use them. We do not permit cookies, pixels, or other tracking technologies to disclose your protected health information to third parties for their own purposes. Where a tracking-technology vendor may receive protected health information, we permit that only under a business associate agreement and on a HIPAA-permitted basis, or with your written authorization. When signing up for pharmacy services or ordering prescription drugs, we may ask you to provide us with certain personal and/or health-related information. This information is subject to the Revelation Pharma HIPAA Privacy Notice, which more specifically describes how we may use or disclose your protected health information. Your protected health information is governed by that Notice, not by this Privacy Policy.

We may collect personal data from you from third parties. We may use analytics service providers to analyze how you interact and engage with the Revelation Pharma Digital Services, or third parties may help us provide you with customer support. When you sign up for an account with us using your calendar provider, such as Google Calendar or Office 365, we may import the contents of your calendar account, including any calendars to which you have access, to your Revelation Pharma Digital Services account. Additionally, if you register an account that is part of an organization, we may collect the names and email addresses affiliated with your organization.

HOW WE USE YOUR INFORMATION

We may use the information we collect from you when you register, post or purchase products, respond to a survey or marketing communication, or use other Revelation Pharma Digital Services features in the following ways:

To personalize your experience and to allow us to create, develop, operate, improve, and deliver the type of content and product offerings in which you are most interested.

To allow us to better service you in responding to your customer service requests.

To quickly process your transactions.

To verify and validate your identity.

To vendors whose products or services you have requested, in order to deliver those products or services to you.

To our subsidiaries or affiliates, strategic partners and to third parties we engage to provide services on our behalf, such as website hosting, credit card payment processing, order processing, delivery, etc. We prohibit these parties from using your personal information for any other purpose.

In the event of the sale or transfer of Revelation Pharma, of one or more of our business units or of some or all of our assets, or in the context of some other business acquisition transaction. Any protected health information transferred in such a transaction will remain subject to the HIPAA Notice of Privacy Practices and applicable law.

In response to a subpoena or court order, or a request from a law enforcement or other government agency; to establish or exercise our legal rights, or to defend against claims; or to protect the safety or security of the public or of users of the Revelation Pharma Digital Services.

We may forward your prescription information to your insurance plan, so that we may handle reimbursement of your prescription purchase for your convenience.

We can disclose aggregate information about users of the Revelation Pharma Digital Services to our service providers, partners, advertisers or others, but such information will not include personal information.

COOKIES, ETC.

Cookies are small files that a site or its service provider transfers to your computer’s hard drive through your web browser (if you allow) that enables the site’s or service provider’s systems to recognize your browser and capture and remember certain information. However, to the extent that Internet Protocol (IP) addresses or similar identifiers are considered personal information by local law, we also treat these identifiers as personal information. Similarly, to the extent that non-personal information is combined with personal information, we treat the combined information as personal information for the purposes of this Privacy Policy.

Revelation Pharma and its partners may use cookies and other technologies to remember personal information when you use our website, online services, and mobile application. Our goal in these cases is to make your experience more convenient and personal. For example, knowing your first name lets us welcome you the next time you access the Revelation Pharma Digital Services. Knowing your country and language helps us provide a customized and more useful experience, and provide you with better customer service.

You can help ensure that your contact information and preferences are accurate, complete, and up to date by logging in to your account via the Revelation Pharma Digital Services and reviewing your personal account and profile information. You can correct or update your information by re-registering for the services, updating your account information via the Revelation Pharma mobile application or website, or by contacting us by email privacy@revelationpharma.com. It may take us up to 30 days to process your request.

For other personal information we hold, we will provide you with access for any purpose, including to request that we correct the data if it is inaccurate or delete the data if Revelation Pharma is not required to retain it by law or for legitimate business purposes. We may decline to process requests that are frivolous/vexatious, jeopardize the privacy of others, are extremely impractical, or for which access is not otherwise required by local law.

We are committed to protecting your privacy. Mobile information, including phone numbers and SMS registration data, will not be shared with third parties for marketing purposes. We maintain strict policies and proactive measures to prevent any unauthorized data sharing. You have the right to opt out of messaging campaigns at any time. CHILDREN’S PRIVACY

The Revelation Pharma Digital Services are intended for adults. We understand the importance of taking extra precautions to protect the privacy and safety of children using the Revelation Pharma Digital Services. Accordingly, we do not knowingly collect, use or disclose personal information from children under 16, or equivalent minimum age in the relevant jurisdiction, without verifiable parental consent. The Revelation Pharma Digital Services may contain links to third-party websites, products, and services. Our services may also use or offer products or services from third parties. YOUR PRIVACY RIGHTS (CALIFORNIA AND OTHER STATES)

California Civil Code Section § 1798.83 permits users of the Revelation Pharma Digital Services that are California residents to request certain information regarding our disclosure of personal information to third parties for their direct marketing purposes. Revelation Pharma does NOT disclose your personal information to third parties for their direct marketing purposes.
